Job description: Position: BHS Case ManagerPosition type: Per Diem/ Fee for Services/full time/part timeRate: $25-$30 per hour based on experienceJOB SUMMARY:The Case Manager works with eligible families with moderate to minimal case management requirements to assist them in identifying their strengths and needs. The Case Manager works with the family to develop an individualized service plan that includes appropriate services and supports that meet the needs of the child and their family. The Case Manager informs the family of services and supports and assists, when needed, in coordinating services and supports.ESSENTIAL JOB FUNCTIONS:1. Adopts, implements and promotes agency values and philosophy in the course of employment with the agency.2. Maintains a minimum caseload of clients required by the agency.3. Assesses the client’s strengths and needs3. Facilitates in developing an Individual Support Plan, 90 days review and annual plan4. Informs children and families of the rights of parents as recipients of mental health services.5. Coordinates services for the client evaluates client progress and satisfaction with services and monitors the effectiveness of services in meeting the needs of the client.6. Maintains complete records for all clients on caseload.7. Advocates for the protection of client rights and entitlements as outlined in the Rights of Recipients of Mental Health Services8. Maintains ongoing relationships through home visits, phone contacts and Collaboration with clients, families and service providers.9. Completes any required documentation in a timely manner (i.e. client record information, evaluation data, progress notes, request for funds, etc.).10. Strives to empower families with the skills to navigate the mental health system.11. Complies with all agency case management standards.12. Must be willing to work with clients with diverse cultures.OTHER RELATED DUTIES AND RESPONSIBILITIES:1. Represents the agency on committees and at meetings throughout the services area to promote the program, its work and the fundamental values, as approved by the supervisor.2. Responds to acute, unexpected emergency situations.3. Maintains client and family confidentiality in compliance with State and Federal laws.4. Prepares and submits reports summarizing activities upon request.5. Participates in weekly supervision, either group or individual6. Acts as a consultant to the agency as a parent of a child with special needs, providing feedback to case managers and administration around programs and training.7. Assists in training new staff as needed8. Attends mandatory weekly supervision with the supervisor.SPECIFICATIONS/QUALIFICATIONS:– MHRT/Certification (training provided)EDUCATION/TRAINING– Undergraduate degree in the related field
– 2 years experience in related field or cultural relevancyJOB-RELATED EXPERIENCE:1. Parent of a child with special needs. If not a parent of a child with special needs must have a four-year degree with at least one year of relevant experience working with families and children.2. Ability to facilitate group discussions.C. SPECIAL SKILLS:1. Sensitivity to others and comfort working with children with mental illness, their families and other services providers.2. Ability to work within a community agency environment to provide effective, high-quality services to children and their families in a manner that respects the dignity and concerns of each individual.3. Knowledge of particular disorders of childhood and childhood development4. Working knowledge of the range of services needed by families with children with mental health issues and/or mental retardation or other developmental delays.5. Advocacy, both cases, and systems.6. Ability to learn and develop good interviewing and assessment skills.7. Demonstrated proficiency in the use of computersCOGNITIVE REQUIREMENTS:1. Strong written, oral and interpersonal communication skills.2. Ability to learn to assess client needs and translate those needs into appropriate support plans.PHYSICAL REQUIREMENTS:1. Ability to maneuver in a variety of home settings and weather conditions.WORK ENVIRONMENT:1. Substantial travel to client’s homes.2. Staff and client with various cultures, religion and cultures may be working in the same building3. Working with clients who have mental health issues or traumatic experiencesOTHER:1. There may be some evening or weekend work based on client needs2. Reliable transportation and valid State of Maine driver’s license and auto insurance is necessary – Your vehicle must have a minimum of $100,000/$300,000 liability coverage to transport clients.Benefits
